Our online scheduling tool allows you to request appointments with providers at Aleman Family Care Center. Submitting an appointment request does not guarantee that an appointment will be confirmed. Our staff will contact you to confirm your appointment. For same-day sick visits, please call our office directly at (414) 231-9501.
For emergencies, please call 911 or proceed to your nearest emergency room. Do not use the online scheduling system for emergencies.
Access to and use of our patient portal (Elation Passport) is governed by separate terms and conditions established by Elation Health. By using the patient portal, you agree to Elation Health's terms and privacy policy in addition to these Terms.
THE SITE IS PROVIDED ON AN "AS IS" AND "AS AVAILABLE" BASIS WITHOUT ANY WARRANTIES OF ANY KIND, EITHER EXPRESS OR IMPLIED.
